New Patient Registration

How to register

Once it has been established that the individual lives within the Practice Area, they are asked to provide the documents listed below. A map showing our boundary (7 mile radius) can be obtained from reception.

Should you move to a new address which falls outside the Practice boundary you will be required to register with a practice within your new area and within 1 month. BSO are notified of any change of address and will contact patients to notify them of their removal from the practice list.

Please note that all patients wishing to register with the practice must provide a brief summary printout from previous GP. This must include past and present medical problems, medication, allergies and vaccinations. We are unable to register or prescribe medication until this information is received. A summary is easily obtained from your previous Practice.

Please complete the New Patient Questionnaire and either the HS200 or HSCR1 (whichever is applicable):

  1. HS200 / HSCR1
  2. New patient questionnaire (RTF, 37KB)
  3. Photographic ID (Passport, Driving Licence)
  4. Proof of address (Utility Bill, Bank Statement)
  5. Brief Summary from previous GP

Patients registering from one GP practice to another either within Northern Ireland or transferring from Great Britain

HS200 form - to be completed by anyone joining the practice from another practice WITHIN Northern Ireland

To register with the practice please bring your medical card to reception. You will also need to complete the HS200 form or call reception. Please note you will also need to provide photographic ID on registration.

Application to register with a GP Practice for eligible visitors or persons taking up ordinary residence in Northern Ireland.

HSCR1 form - to be completed by anyone joining the practice from OUTSIDE Northern Ireland


If you wish to register you must complete the appropriate form and provide the relevant supporting documentation. You can print the HSC/R1 form or you can call into reception to pick up a form.

If you require further information please speak to one of our receptionists.

All new patients will be given an appointment to see the Doctor and the Practice Nurse. Please complete the questionnaires above and bring it to your new patient medical appointment.
